Register – Register with My Aged Care and request an aged care assessment
Aged Care Assessment – Aged Care Assessment and approval for low care or high care residential respite or permanent residential aged care
Referral codes – Referral codes received from the Aged Care Assessment. This allow you to access government-funded residential aged care

Compare – Compare residential aged care facilities based on the clients care needs, financial position and personal circumstances
Choose – Client to choose the residential aged care facility option that best suits them

Step 3 Move In Day
Admission – Support client to complete admission paperwork and organise the “move in day”
